  • Performance of a vitrectomy in a case with difficult posterior vitreous detachment.
  • Subretinal infusion of balanced salt solution (BSS) with a 41-gauge needle to detach the retina.
  • Removal of residual vitreous cortex from the detached retina.
  • Gentle separation of the retina with stuck CNV, using different instruments.
  • Detachment of the CNV from the RPE, followed by a diathermy at different spots.
  • Full separation of the CNV from the retina, followed by aspiration with a vitrectomy probe.
  • Isolating a patch of RPE and choroid from the temporal retina.
  • Defining the outlines of the patch via diathermy. Cutting out the patch with vertical scissors.
  • Placing the patch under the fovea.
  • Reattachment of the retina by gently moving the PFCL out from under the retina to the epiretinal plane.
  • The patch is now perfectly centered under the fovea.
